OHIO PUBLIC SCHOOL SYSTEM

In order to register your child or children in Ohio public schools, you will need to bring the following information and documentation: Certified stamped-seal birth certificate and social security card for the child, custody papers if applicable, proof of residency, and Ohio health history forms. Residency proof can take the form of a recent utility bill, lease agreement, mortgage statement, or other legal documentation received within the last 60 days.

Kindergarten attendance and kindergarten screening are required in Ohio. Children must be at least five years of age on or before September 30th of the enrollment year in order to register. You will need a physician-signed immunization record indicating doses and dates. Other important documents include transcripts from the previous school attended or the name and address for ordering past records as well as the contact information for your child-care provider. Kentucky Public School System

Children must be at least five years of age by October 1 of the enrollment year in order to register for kindergarten in Kentucky. Parents must provide an official copy of the student’s birth certificate, and the child’s social security card. In order to comply with Kentucky health laws and regulations, all new registrants must bring a physician-signed immunization certificate and proof of a physical exam. Students who transfer from out-of-state districts must also comply with the state’s immunization laws. All school physical exams must be complete and have been signed by a physician no more than one year prior to the students’ first-time enrollment. First-time enrollees must also have an eye exam.  No grace period is allowed for compliance on health records.

Proof of residency can take the form of a recent utility bill, lease agreement, mortgage statement, or other legal documentation received within the last 60 days. Be prepared to provide legal custody papers when applicable to your situation. Registration is held at the school where the child will be attending, although it is not necessary that the child be present at registration. INDIANA PUBLIC SCHOOL SYSTEM

Children must be at least five years of age by July 1 of the enrollment year in order to register for kindergarten. First graders must be a least six years of age by the same cut-off date. Some school districts will allow early registration on a case-by-case basis. Legal birth certificates and official immunization records are required for all new students. You must also bring proof of residency and a current address in the form of utility bills, rent receipt or mortgage papers. Other required information or documents include student’s social security number, guardianship papers if application and records from the previous school or a registrar contact. Dental or health exams may be required according to district guidelines. New students have a 30-day grace period in which to comply with all regulations.
