Registration Requirements
Students registering for kindergarten must be five years of age by September 30 of the enrollment year. Documents needed for registration include the child’s official birth certificate and Social Security card, and the Louisiana Child Health Record Form with the immunization and physical exam sections completed and signed. New residents must verify their address in the form of a lease, mortgage, or utility bill in a custodial parent’s name. Transcripts from the previous school will also be helpful.
