ACADEMICS / PUBLIC EDUCATION SYSTEM

In order to register your child or children in Ohio public schools, you will need to bring the following information and documentation: Certified stamped-seal birth certificate and social security card for the child, custody papers if applicable, proof of residency, and Ohio health history forms. Residency proof can take the form of a recent utility bill, lease agreement, mortgage statement, or other legal documentation received within the last 60 days.

Kindergarten attendance and kindergarten screening are required in Ohio. Children must be at least five years of age on or before September 30th of the enrollment year in order to register. Also required for school registration is a physician-signed immunization record indicating the dates for four DPT or DtaP shots, three polio vaccines, two MMR (measles, mumps, and rubella) vaccines, and three Hepatitis B shots. Other important information to bring to registration includes transcripts from the previous school attended or the name and address for ordering past records as well as the contact information for your child-care provider.
